This is for a provider that is just starting out with a new Tax ID and one performing provider. A new Medicare PTAN number is required for every practice that you are with. Your number changes per practice.
If you have practice locations in separate counties it is required that a new enrollment at an additional fee will be completed by county localities as decided by CMS Medicare. (This includes Dallas, Tarrant, Harris, Travis, Jefferson, Brazoria, Galveston, and Rest of the State [Other]).
*If you have additional providers, please note that there will be another enrollment per provider*
- This includes Texas Medicare enrollment for the group and one provider reassignment.
- A group Medicare number (PTAN) and individual provider PTAN will be issued.
- This cannot be started until 60 days prior to the start date of practice.
