Follow these steps to join today:

  1. Request and application above:
    • Be sure to complete all components of the application.

  2. You will receive an email notification from our credentialing team regarding your application, with an acceptance, missing information, or other notices within 5 days of submission.
  3. Our Board of Directors meets on the third Monday of every month unless otherwise stated for Holidays or office closures.
    • Cut off to receive a completed and accurate application as well as required documents is 10 days before the Board meets.
      • ie. Board is Monday, July 18th, all application packets are due on Friday, July 8th by the end of business
  4. We offer expedited membership credentialing for an additional fee if an application is received after the due date to make it to the month’s Board.
  5. You will receive an email of Membership approval by the following Tuesday and instructions to view the provider Web Portal, where you can find and edit provider enrollment and demographics.
  6. Submissions to the networks are completed 10 business days after Board and then effective dates for those networks are received within 30-120 days, dependent on the networks.
    • NOTE:¬†You are required to have a PTAN (Medicare Number) to be submitted to the Medicare Advantage managed care networks, and a TPI (Texas Medicaid ID Number) to be submitted to the Medicaid managed care networks, like CHIP, STAR, STAR+PLUS, and STAR Kids, and a BCBS Provider ID to be submitted to all BlueCross BlueShield and TriWest Plans. Once these ID Numbers are received, they will then be submitted to the networks on the next closest submission.

  7. Please keep us updated on all demographic changes via the WebView web portal to maintain accurate data with the networks. We have a dedicated submission each month for demographic changes to go out to the networks.
  8. You can contact for any out-of-network issues, claim denials due to network status, or other network-related questions. Please include the Provider’s Full Name, NPI, Tax ID, and Network with Line of Business that you have inquiries about to best assist you with your questions.
  9. There are annual dues that are due in September of every year, please pay timely to avoid additional late fees.
  10. Reappointment applications and fees are due every 3 years for every practicing TIN that you are with.

    NOTE: There are no refunds issued for any services that have been started or completed.
