This is for a provider that is joining an established group. The group already has a group Group PTAN. A new Medicare number is required for every practice that you are with. Your number changes per practice. If you have practice locations in separate counties it is required that a new enrollment be completed county localities as decided by CMS Medicare (This includes Dallas, Tarrant, Harris, Travis, Jefferson, Brazoria, Galveston, and Rest of the State [Other]) and will require a separate fee.
- This includes Texas Medicare enrollment for one provider reassignment.
- An individual provider PTAN will be issued.
- This cannot be started until 60 days prior to the start date of the provider at the practice
