Application Process

Provider Requirements
All providers must hold a valid Texas medical license.
All providers must have malpractice insurance coverage with their name on it.
* NPs and PAs are required to be board certified.

TIOPA can submit select payer and government enrollment applications on your behalf, including Medicare PTAN, Texas Medicaid/TMHP, BCBS Provider ID (PFIN), CAQH ProView, Railroad Medicare, hospital privileges, and facility credentialing.
