Reactivate Application Fee


Fee charged to reactive an application that was received and staff began the process of getting the provider “board ready”.  If something comes up which keeps the provider from going to board and they are not able to get ready within the 120 day time frame, the file must go into “inactive” status.




If a provider takes longer than 120 days to get credentialing information to us- we put them on “inactive applicant status”.  In order to reactivate, there will be a charge due to credentialing having to redo verifications and other application documents.
